The PS2 version also supports the ability to map custom faces to the player's skaters. The game also introduces new options for maneuverability, including disembarking from the board (for climbing up to places normally unreachable by skating, or for better preparation mid-combo), wallplanting/wallpushing, acid-dropping onto ramps, and driving certain vehicles. It also adds more tricks per skater, including unlockable Special Trick slots and "double-tap" Grinds.

Tony Hawk's Underground, like previous installments in the series, features crossover characters from different franchises as hidden playable skaters: Gene Simmons (the bassist from the band KISS) and Iron Man (a superhero from the Marvel franchise of the same name).

The game also received a handheld release by Vicarious Visions for the Game Boy Advance, released alongside the other consoles.
